Before you begin
What is an Adult Social Care referral?
When you make a request for social care services to Leeds City Council we call it a 'referral'. Online referrals should not be used in emergency situations as there may be a delay in the referral being processed. You can use this form to create a new referral if you are worried or concerned about an adult who needs support:
- With personal care such as washing, dressing and toileting
- To stay safe around the home including equipment and alarms
- To access the community, leisure or work
- To maintain relationships
- Support for unpaid carers
Make sure you have spoken to the person to ensure they want help from us. We may not be able to help someone who does not want it.
Please read the following information before making a referral
For urgent support in relation to a person's mental health contact the Single Point of Access (Crisis team) 0300 300 1485.
If you are concerned about someone's long term mental health needs, please refer to their GP unless there are other care and support needs or you feel they are at significant risk.
If you or someone you know are in immediate danger please contact the police on 999.
For housing needs, homelessness or risk of homelessness, please visit the housing needs page on our website (opens in new tab)

If you are concerned about domestic abuse and the person does not have a care and support need, please visit the Leeds Domestic Violence Service website (opens in new tab)

If you have a query or concern about an existing referral please get in touch on 0113 222 4401. Our working hours are Monday to Thursday, 9:00 AM until 5:00 PM and Friday from 9:00 AM until 4:30 PM except for Bank Holidays.
